Position Overview

We are seeking experienced and compassionate Field Registered Nurses (RNs) to perform in-home visits across all NYC boroughs . As an FFS RN, you will play a vital role in patient care, assessments, and care coordination—helping clients live safely and comfortably in their homes.

Key Responsibilities

• Conduct initial and follow-up in-home nursing assessments
• Develop and update individualized care plans
• Educate patients and families on disease management and self-care
• Supervise and instruct caregivers and home health aides as needed
• Complete timely and accurate documentation in compliance with regulations
• Communicate effectively with interdisciplinary team members

Qualifications

• Active New York State RN license
• 1–2 years of experience in CHHA or long-term care preferred
• Strong clinical assessment and documentation skills
• Professional liability insurance required
• Reliable transportation and willingness to travel between boroughs

Compensation (Per Visit Rates)

• Initial Assessment Visit : $130
• Reassessment Visit : $100
• Resumption of Care Visit : $100
• Supervisory Visit : $80
